Application Procedure:

• Complete the following application form and submit for approval
• Submit a copy of your driving record (available at the Ministry of Transportation Office – (925 Dundas St. Woodstock ON)
• Apply for Taxi Driver’s License at Woodstock Police Service (615 Dundas St. Woodstock, ON)
